Where should I apply for a name change if I was born outside Southern Jutland?

If you were born outside Southern Jutland, you apply for a name change at your parish's church office.

Can I apply for a name change if I am getting married?

If you are getting married and at the same time want a name change, you must apply for it here.

 Note: If you were born in Southern Jutland, you apply for a name change at the municipality where you lived when you were born. Use the self-service link at the top. If you were born outside Southern Jutland, you apply for a name change at your parish parish's church office. Here you can find your parish. The application must be sent no later than 15 days before the wedding.
