Your Dialogue is a secure, digital platform for those of you who receive help or are in a process with the municipality - whether it concerns home care, rehabilitation or the social area.

The purpose of the solution is to give you greater insight into your own medical record and make it easy to communicate securely with the employees you have agreements with. You can access Din Dialog from both computer, tablet and smartphone.

What is Your Dialogue and what can I see?

Your Dialog is your personal entry point to the information that Sønderborg Municipality has registered about your electronic care record.

Through the platform you get access to:

• Journal and efforts: See selected information and the assistance efforts that have been initiated in your case.
• Calendar overview: Get an overview of your appointments, see who is coming to visit, and cancel appointments digitally.
• Aids: See an overview of the services and aids you receive from the municipality.
• Secure messaging feature: Write directly to the entity you are affiliated with and read past correspondence.
• Own measurements: Possibility to enter data such as blood pressure, pulse or blood sugar after agreement with the staff.

What can I use the Din Dialog solution for? 

Your Dialogue is designed to make your contact with the municipality easier and more manageable:

• Communication: You can write secure messages directly to the device you are connected to. You can expect a response within three business days.
• Agreements: You can see times for visits (e.g. cleaning or training) and the name of the employee who is coming. It is also possible to cancel appointments directly in the system.
• Measurements: After agreement with the staff, you can enter measurements yourself, such as blood pressure or blood sugar, so that the staff can follow your progress.
• Video calls: You have the opportunity to participate in scheduled video calls with municipal employees.

Who can use Din Dialog in Sønderborg Municipality?

The solution can be used by anyone over the age of 18 who has an active case or is receiving help within, among other things:

• Home care and home nursing.
• Training, brain injury coordination and prevention.
• Nursing homes and residences.
• Social psychiatry and the disability field.
• Substance abuse center and preventive efforts.
• Mobility aids 

Can my relatives have access?

Yes, it is possible to give your loved ones power of attorney to use Din Dialog on your behalf. With a power of attorney, your relative can log in with their own MitID and help you keep track of appointments, read information and write to the staff.

If the person is exempt from Digital Post, it will arrive as regular mail.

How is the security and data processing?

To protect your personal information, you must always log in with MitID. All information is treated confidentially, and your messages or entered measurements are stored directly in the municipality's care system. For your security, you will be automatically logged out after four minutes of inactivity.
